I have given my consent to Greenville College (GVC) and its designated representative, in compliance with Republic Act No. 10173 otherwise known as the Data Privacy Act of 2012, in cooperation with National Privacy Commission (NPC) to collect, process, and store my personal information in relation to my admission, enrollment, and education in the said institution. Personal information includes the following, but is not limited to, the student’s name, address, contact numbers, email address, names, work and contact details of the parents/guardians, nationality, religion, birthday, health records, academic information such as grades, attendance, conduct, learner’s reference number, and other information necessary for the basic administration and instruction. I understand that all personal information will be protected and cannot be disclosed without my consent and that the collected and processed information will be utilized by GVC, as an educational entity to pursue its legitimate interests. I am aware that GVC may share such information, as provided by the law and as part of the extension of privileged communication, to affiliated/partner organizations and with government agencies as part of its contractual obligations and pursuant to legal processes.
